Memory Coach

JOB SUMMARY

The Memory Coach is a specialized clinical support role designed to bridge the gap between medical diagnosis and daily functional engagement for patients with Alzheimer’s, dementia, and other cognitive impairments. The Coach utilizes evidence-based behavioral techniques and cognitive stimulation to slow functional decline and manage the behavioral and psychological symptoms of dementia (BPSD). This role requires a unique blend of clinical empathy, behavioral observation, and the ability to implement structured non-pharmacological interventions.

CORE RESPONSIBILITIES

  • Cognitive Stimulation Therapy (CST): Lead individualized and small-group cognitive exercises, including reminiscence therapy, sensory integration, and validation therapy.
  • Behavioral Intervention: Identify environmental and internal triggers for agitation, sundowning, and anxiety. Implement calming strategies and redirection techniques rooted in ABA or psychiatric tech principles.
  • Family & Caregiver Coaching: Educate families on communication 'dos and don'ts,' home safety modifications, and techniques to manage difficult behaviors without escalating conflict.
  • EMR Documentation: Maintain detailed clinical notes in eClinicalWorks (eCW) regarding patient participation, behavioral trends, and progress toward non-pharmacological goals.
  • Safety Oversight: Monitor patients for changes in gait, mood, or cognitive status and report immediate concerns to the LCSW or Physician.

QUALIFICATIONS & EXPERIENCE

Candidates must demonstrate a background in one of the following specialized areas:

  • Memory Care / Skilled Nursing: Experience as an Activity Assistant or Director in a locked memory care unit.
  • Behavioral / Mental Health: Prior experience as a Behavioral Health Technician (BHT) or Mental Health Technician (MHT).
  • Psychiatric Care: Experience as a Psychiatric Technician in an inpatient or residential setting.
  • ABA Background: Experience as an ABA Technician or Registered Behavior Technician (RBT), specifically working with individuals with Autism or developmental disabilities.
